چکیده

Introduction Heel ulceration associated with calcaneal osteomyelitis is a very difficult pathology to treat and often results in below knee amputation. Surgical excision of the infected bone and ulcer via calcanectomy has been shown to reduce amputation and mortality rates. The purpose of the study was to review our experience with this procedure and examine factors that might influence outcome.

Surgical management of a diabetic calcaneal ulceration and osteomyelitis with a partial calcanectomy and a sural neurofasciocutaneous flap

The treatment of calcaneal osteomyelitis in diabetic patients poses a great challenge to the treating physician and surgeon. The use of a distally based sural neurofasciocutaneous flap after an aggressive debridement of non-viable and poorly vascularized tissue and bone that is combined with a thorough antibiotic regimen provides a great technique for adequate soft tissue coverage of the heel. ...

Partial Calcanectomy in High-Risk Patients With Diabetes: Use and Utility of a “Hurricane” Incisional Approach

INTRODUCTION Plantar heel ulcers in people with diabetes represent a difficult challenge to the treating physician. They become even more difficult with underlying osteomyelitis. When this infection is in the calcaneus it typically results in a partial or total calcanectomy or even more frequently, high-level amputation. METHODS In this article, we describe a novel serpentine incisional appro...

Gaenslen’s Split Heel Approach for the Treatment of Chronic Osteomyelitis of the Calcaneus: A series of three cases

Background: Osteomyelitis of the calcaneum with non-healing ulcer is an old but challenging entity to orthopaedic surgeons. Acute osteomyelitis is treated by intravenous antibiotics and drainage. There are different surgical procedures to manage chronic osteomyelitis of calcaneum i.e. partial, total calcanectomy, split heel approach and vascularized flaps to cover the ulcer. In this reported se...

Treatment of chronic heel osteomyelitis in vasculopathic patients. Can the combined use of Integra® , skin graft and negative pressure wound therapy be considered a valid therapeutic approach after partial tangential calcanectomy?

Osteomyelitis of the calcaneus is a difficult problem to manage. Patients affected by osteomyelitis of the calcaneus often have a below-the-knee amputation because of their comorbidity. In this article, we present seven cases of heel ulcerations with chronic osteomyelitis treated with Integra(®) Dermal Regeneration Template, skin graft and negative pressure wound therapy after partial tangentia...

Lower Extremity Function following Partial Calcanectomy in High-Risk Limb Salvage Patients

Partial calcanectomy (PC) is an established limb salvage procedure for treatment of deep heel ulceration with concomitant calcaneal osteomyelitis. The purpose of this study is to determine if a relationship exists between the amount of calcaneus removed during PC and the resulting lower extremity function and limb salvage outcomes. Consecutive PC patients were retrospectively divided into two c...
